Polevault Verification Forms

Reminder: As most of you know, at the check-in for the Polevault your athletes will need to turn in a pole verification form. They will not be allowed to take any jumps (even warm-up jumps) before turning in this form. The form should clearly state your athlete's Name, weight and the suggested weight rating(clearly printed on the label at the end of the pole) of the smallest pole that vaulter will be using. The officials may or may not have any forms but THEY are not required to provide them. [b]It will help GREATLY if you can have these forms printed, filled out and signed by a Coach [i]before[/i] you arrive to the event.[/b] I did a google search and came up with this link for a PDF of the form used at State last year. It seems to meet the necessary requirements. Thanks for this reminder. I will have blank copies of something similar to this at the meet for coaches to fill out this week.
